Use cross fades for audio muting control during Audio Follow Video（AFV).

Bug fixes

Fixed a bug in the console mixer where USB audio input level may become unadjustable.
Removed Line In / Mic In mute configuration items from the console menu. These items were duplicate since they are configurable in the audio mixer menu.
Fixed a bug that the HDMI audio streams are not received at correct sampling rate for first few seconds, if the sampling rate is 44.1 kHz or 32 kHz.
